KnowMyMeds Provider Gateway for Primary Care  and Home Health Care Providers

The KnowMyMeds™ solution is a simple, secure web-based tool to help you manage complex medication regimens in older adults or other high risk patients.

• KnowMyMeds is more than a website for patients to enter their medication data.  The clinically-validated KnowMyMeds Rules Engine identifies potential risks based on patient-reported data on all medications including prescriptions, OTC drugs and supplements as well as symptoms and conditions, and observations of daily living (ODL), thus providing a broader perspective on how patients are self-managing their health.  Electronic data interchange allows for automatic reconciliation of your EHR with patient-reported data from KnowMyMeds.
• KnowMyMeds is simple for patients to use, easy to incorporate into your existing workflow, and can help you improve outcomes.  In a clinical research study, patients who used the  KnowMyMeds technology improved conditions like high blood pressure with fewer medications and/or lower doses than patients in a control group, and also reported greater satisfaction and motivation to adhere to their care plans.
• Using the KnowMyMeds solution is a billable to many payers as a Risk Assessment and Medication Therapy Management/Reconciliation service.
• The AdhereTx Provider Gateway quick search function lets providers find specific patients or query the population summary for specific risks, symptoms, conditions, behaviors or medications.  Searching and automatic identification of Beers criteria medications also support registry reporting activity for seniors on complex medication regimens.
• Know My Meds Provider Gateway offers providers a way to demonstrate significant “meaningful use” criteria independently, or in tandem with their current EHR.

Technology and Security

The AdhereTx solution is a  robust, enterprise-scale Software-as-a-Service solution hosted on enterprise class, name-brand cloud servers. AdhereTx applies  best practices embodied in HIPAA and the PCI Data Security Standard. Access to patient data will be fully auditable, accessible to clinical users based on an access control system that tiers information delivery on a need-to-know, authorized basis.

Interoperability

Interoperability is key to AdhereTx solutions delivery. Our solutions are designed to interoperate with industry leading EHR and PHR systems via data change standards (e.g. ASTM CCR, CCD/C32/HL7) and via custom data interchange interfaces.  Additional specific EHR/PHR solutions integration that are not standards-based are prioritized based on customer requirements.  We work with solutions delivery partners, expert in EHR implementation and integration certified partners as needed. Additional data integrations include biometric monitoring devices according to the Continua open standard, mobile and other solutions.
