Know My Meds is a web-based solution that is the product of many years of research and clinical validation at the University of Connecticut School of Nursing. It is designed to be used by older adults and/or their caregivers to keep an up to date record of their medications, including the prescription drugs, OTC and supplements they are taking. The program also allows them to report relevant information about their daily living, as well as new symptoms or conditions that may arise. A SmartList™ is produced that organizes all of this information, provides questions for the patient’s health care provider, and can be automatically shared with family members or others designated by the patient.
The program uses AdhereTx’s SmartData Capture™ in the form of a “motivational” interview that can be accessed from any browser (www.knowmymeds.com) and completed either by the patient or with the assistance of a caregiver. The initial assessment requires no more than a half hour. Regular updates to the program can be accomplished in just a few minutes. Information is captured in 3 areas: conditions of daily living, symptoms and conditions, and medications.
